CORI Checks and Fingerprinting

Monomoy Regional School District is committed to the safety and security of its students, teachers, staff, and community. As such, the district utilizes the Criminal Offender Record Information (CORI) system, the Sex Offender Registry Board (SORB), as well as fingerprinting for those individuals who work with students. CORI is a record of all criminal court appearances in Massachusetts for a particular individual, including arrests, convictions, dismissals, and serious violations. The SORB registers and classifies convicted sex offenders according to their risk of re-offense and the degree of danger they pose.

Any parent, chaperone, volunteer, staff, substitute, coach, extracurricular advisor, or seminar instructor who will be working with students (one time or on a regular basis) must have a CORI and SORB check performed.

Please complete the form and return to the main office of the appropriate school. You will need to bring your driver's license, which must be photocopied and attached to the form. You can also email the completed form and copy of your license to Joy Jordan at jjordan@monomoy.edu for processing. CORI/SORB checks must be resubmitted every three years. Please note that it can take up to a week to process CORI/SORB checks.

Anyone who is regularly working with students in our district, including teachers, staff, substitutes, coaches, and bus drivers, must be fingerprinted in addition to undergoing a CORI/SORB check.
