Below is the list of things that will need to be completed before the first day of practice in order for student-athletes  to participate:
1.) Updated Physical on file with the school(Check the date on aspen, under athletics)
2.) Register on Family ID  
3.) Complete the NFHS concussion in sport course and email our Athletic Trainer Taylor Murray, tmurray@monomoy.edu, your certificate of completion.

If you have completed this for another sport THIS school year, you do not need to complete it again
4.) Complete the  Implicit Bias Course and email me your certificate of completion to our Athletic Trainer, Taylor Murray tmurray@monomoy.edu. If you have completed this for another sport, you do not need to complete it again.

5.) Updated ImPACT test. ImPACT testing will be held after school closer to the start date or you can take the test during Jawsomes called treatments. Make sure you bring your computer with you during Jawsome. Each ImPACT test once taken is valid for two years.  If you are unsure about expiration dates, please email me and I will let you know where you stand.
